What is Patient Referral Program?
Refer a friend or family member to ProCare Family Dental and both you and your referral can enjoy great bonus rewards through our Patient Referral Program.
Here’s how it works: Pass our name along to people you know, and make sure they mention you when they schedule their visit or fill out the form below. After they complete their new patient appointment, which includes a comprehensive exam, cleaning (in absence of gum disease), and necessary x-rays, you’ll both receive a $50 Dental Credit towards future Dental Treatment. You can refer as many patients as you want, but we limit the Dental Credit to one per immediate family (spouses and children). Patients can earn unlimited Dental Credits by referring friends and acquaintances, as well as extended family members such as aunts, uncles and cousins.

Referee must be a new patient and mention the referrer’s name when booking their appointment.

Referee must complete the necessary dental treatment in order to secure $50 off their next appointment.

There is NO LIMIT on the number of members that can be referred and therefore the number of $50 vouchers you can receive.

Referrers can use their $50 off voucher at their next visit, with vouchers needing to be used within 12 months of issue. Referrers should mention that they will be using a voucher at the time of booking, and vouchers can only used once.

Voucher cannot be redeemed for cash, and any remaining balance will not be refunded.
